We've had a few users report missing panels or contextual panels one command behind in Revit 2012 and 2013. The symptom looks similar to the image comparison below:

This behavior can be reproduced when a pre-release or beta version of Microsoft® .NET® is installed, typically the Microsoft .NET Framework 4.5 Beta.
If you are experiencing this behavior with Revit 2012 or 2013 we recommend uninstalling .NET Framework 4.5 Beta from the Windows Control Panel and reinstalling the latest .NET 4.0 release.
After .NET Framework 4.5 is uninstalled you can reinstall \ repair .NET 4.0 from the Revit product media or download from Microsoft directly:
